This 43-year-old patient from Beverly Hills, CA complained of dark hollows under her eyes that were making her look tired. Dr. Cassileth repositioned fat in the area to give her a more rested look. This is called a lower eyelid transconjunctival blepharoplasty.

Aftercare protocol
  • Ice packs for the first 48 hours — 15 minutes on, 15 minutes off, while awake
  • Head elevated for sleeping for the first week
  • Lubricating eye drops for the first 2 weeks (transient dryness is normal)
  • No contact lenses for 1–2 weeks; sunglasses outside for the first month
  • Hyperbaric Oxygen Therapy to support healing when recommended

  • Petrolatum-based ointment on the incision lines for the first week
  • SkinCeuticals C E Ferulic on incisions after week 2
  • SPF 50+ on the eyelids and surrounding area for 12 months

Pre-op preparation

What to do before surgery. Specific to this case.

  • Aspirin, ibuprofen, vitamin E, and fish oil discontinued 14 days pre-op to reduce bruising risk. Acetaminophen is allowed.
  • Nicotine in any form, cigarettes, vapes, patches, gum, paused six weeks before and six weeks after surgery. Nicotine narrows blood vessels and slows wound healing.
  • Set up an elevated sleeping arrangement, extra pillows or a wedge, to keep the head above the heart for the first week. Stock soft, low-sodium foods.
  • A driver is required the day of surgery and a responsible adult should stay with you the first 24 hours.

Recovery timeline

Milestones specific to this case. Individual recovery varies.

  1. Day 1–7

    First week is rest, elevation, and ice. Expect the most swelling on day 3, then a steady downhill from day 5 onward.

  2. Week 2

    Sutures or tape come off around day 7–10. Most patients are camera-ready for everyday life by the end of week 2 (lingering swelling, not bruising).

  3. Week 4

    Facial contour 70–80 percent settled. Light cardio, yoga, and gentle facial massage cleared by the surgical team.

  4. Week 6

    Most physical restrictions lift. Return to strength training, full-impact cardio, and overhead lifting.

  5. Month 3

    Final swelling resolves on the deeper tissue planes. Most aesthetic refinements emerge between months 3 and 6.
