This 46-year-old patient from Sherman Oaks, CA initially had saline implants placed nearly 20 years ago. She experienced grade 4 capsular contracture in her left breast, while her right implant had bottomed out. Not only did this result in severe asymmetry, but the hardening in the left breast was very painful. Dr. Cassileth performed a breast implant removal with a complete capsulectomy on the left side and a partial one on the right. She replaced with 310 cc implants, placing them under the muscle with acellular dermal matrix. To give the patient softer, natural-looking breasts, Dr. Cassileth performed bilateral fat grafting.
